Form Builder Wizard - General Info

  1. Super Admin users are required to select a department. This option is not available for Department Admin or Formulator users as it will default to the user's own department.
  2. Enter an intuitive title that describes the form. This will appear as the page and browser title.
  3. Enter an intuitive file name that describes the form. This will be the file that you will access via the URL or Address Bar. All forms are .NET files with .ASPX extentions. Do not include the extension. Valid filenames only include Alphanumeric characters.
  4. Select a template from the list of available templates. Select Default to use the generic template. (Note that Super Admin and Department Admin users can create additional templates. See Skins for more information.)
  5. To include page names in the form (in addition to the Form Name specified), select the first checkbox. This also customizes the Wizard's behavior. So, if there will be just one page, you will not be presented with the option to add pages in the Wizard.
  6. To include section names in the pages, select the second checkbox. This also customizes the Wizard's behavior. So, if there will be just one section per page, you will not be presented with the option to add sections in the Wizard, thus simplifying the process.
